What’s new so far for 2019?  We now have our patient portal up and running!!!

If you would like access to the patient portal be sure to give our staff your email address and we will send you a link.  This is a great way to keep track of past and future appointments, refill medications, send messages to our staff, etc.  Once you have received your log-in information and set up your portal account you can access the portal by just visiting our website and clicking the link at the top right of the page.  For more information please feel free to contact our office.

Happy Holidays from CVI!  Our hours will be changing for the upcoming holidays.  Our office will be closed Monday December 24 & Tuesday December 25th in observance of Christmas.  We will reopen for regular hours Wednesday December 26 – Friday December 28.  On Monday December 31 our vascular lab will be open from 8am – 1pm and our office will be closed on Tuesday January 1st in observance of the New year.  We will then reopen for regular hours thereafter.  We hope you all have safe & happy holidays.
